I'm trying to understand your problem - it looks to me like your bot is functioning properly
it isn't randomly sending the welcome message, it's sending it as a message because your user sent the first message
if you want your bot to be able to answer questions as soon as it receives them, make sure that "Answer on Start Node" is enabled

It's doing this because you've reached the beginning of the conversation again, so it's answering the "SMOKE?" question on the start node, and then executing the rest of the workflow (sending the welcome message)

the bot executes your conversation in a loop, beginning at the start node every time
pay attention to what's written in grey in the emulator
that shows where you are in the conversation, or what's happening at that stage
